Along with county health departments and other health systems, Duke was selected by the state as one of 11 initial distribution sites for the Pfizer vaccine. As a partner in Duke Connected Care, DCC community providers and their staff members who are in Phase 1a can now apply to receive the COVID vaccine through Duke Regional Hospital. If you or your team members are interested in receiving the COVID vaccine and are considered a healthcare worker fighting COVID-19 (Phase 1a of vaccine distribution), please fill out the spreadsheet HERE for your practice/clinic on the DCC Provider Portal. All columns of the spreadsheet must be completed (First Name, Last Name, Email Address, and Contact Phone Number) and include every Phase 1a team member who wishes to be vaccinated. Please email your finalized spreadsheet to dukeregionalhospital@duke.edu at your earliest convenience and include in the email Subject Line the phrase “1a List.”
